Many men on injectable TRT self-administer at home after a clinic shows them how. It's routine once you've learned it, but the details matter for comfort and safety. Subcutaneous vs. intramuscular

Testosterone can be injected into fat just under the skin (subcutaneous) or into muscle (intramuscular). Many modern clinics favor subcutaneous injection with a small needle — it's simpler and comfortable for most men — while others use intramuscular. Your clinic will tell you which is right for your protocol and the needle sizes to use.

Hygiene comes first

Clean technique prevents the most common problems:

  • Wash your hands and work on a clean surface
  • Use a new, sterile needle and syringe every time — never reuse or share
  • Swab the vial top and the injection site with alcohol and let it dry
  • Dispose of needles in a proper sharps container

Rotate your sites

Injecting the same spot repeatedly can cause soreness or lumps. Rotate between approved sites (your clinic will show you which) so tissue has time to recover. Mild tenderness can be normal; spreading redness, warmth, or significant pain is not and warrants a call to your clinic.

Dosing frequency and steadiness

Some men inject weekly; others split into smaller, more frequent doses to keep levels steadier and reduce peak-to-trough swings. This is a protocol decision your clinician makes based on your labs and how you feel — don't change your own frequency or dose without guidance.

When to call your clinic

Reach out if you have persistent pain or signs of infection at the site, notice unusual swelling, or simply aren't confident in your technique. A good clinic expects questions early on and would rather coach you than have you guess.
